The Consumer Directed Personal Assistance Program (CDPAP) is a state-administered Medicaid program designed for Medicaid recipients enrolled in a Managed Long Term Care (MLTC) Plan and requires home care, and who need assistance with activities of daily living.


The consumer or his or her designee, may hire a caregiver or “personal assistant” of their choice, whether it be a friend, family member or loved one, except for a spouse or parent(s) of a child under the age of 21.


The consumer or designee is responsible for recruiting, hiring, training, supervising, and terminating caregivers, and must arrange for back–up coverage when necessary.


An eligible consumer must meet the following criteria and wish to remain in their own home to be considered for CDPAP:


  • Be eligible for Medicaid.
  • Require long term care services, including assistance with home care, personal care, or skilled nursing tasks.
  • Be a full-time resident in New York.
  • Be self-directing or, if non self-directing, have a designated representative;
  • Must be able to participate in care assessments (or choose a designee to do so).
  • Must be able to hire, train, supervise, and terminate caregivers, and must arrange for back–up coverage when necessary (or choose a designee to do so).


Under CDPAP, the consumer may choose, hire, and train a “personal assistant” of their choice as a caregiver. The personal assistant may be a friend or loved one.


To become a personal assistant, an eligible candidate must meet certain New York State requirements:


  • Be at least 18 years old.
  • Eligible to work in the United States.
  • Must be eligible to work government-funded programs, such as Medicaid, Medicare, or other federally funded programs.
  • May NOT be the spouse or custodial parent.
  • Complete required New York State health assessments.


PHCDPAP serves as a liaison between the consumer, their personal assistant, and their MLTC Plan.


Specifically, PHCDPAP will assist the consumer with CDPAP enrollment, provide personal assistants and consumers with required onboarding, serve as the Fiscal Intermediary (FI) to ensure caregivers are paid properly, fairly, and on time, and act as the coordinator between the consumer and the MLTC Plan.
