We serve clients throughout New York City, Nassau, Westchester, Rockland, Putnam, Orange, Dutchess, Ulster, and Sullivan counties.
Our Dedication
Our team of dedicated professionals are prepared to serve as your conduit between your chosen caregiver and Medicaid program. PHCDPAP staff will assist with CDPAP enrollment, care coordination, Fiscal Intermediary services, and onboarding.

We are a multi-lingual company with the resources available to provide services in alternate languages.
